It is vital that the practice has your current address and telephone number in order to be able to contact you in times of emergency or on routine matters.
Therefore if your address or telephone number has changed then the practice needs to be informed in writing as soon as possible. You can either do this via filling out a simple form at the practice or send us an email to info.southgatesbpct@nhs.net or by downloading our Change-of-Address-Forms and sending it to the practice.
Once you have completed the form please return it to the surgery in person or post it to:
